CLS Service Request forms are used for requesting most audio/visual and multimedia support services. These forms are important because they ensure that we have the information we need to complete the request. Examples of services requiring a Service Request include:

Step One 1. Complete a service request form.

Service request forms must be completed on the web. If you wish, you may come by our office for assistance in completing the form, in which case you will be able to use one of our computers to complete the web-based form.

 

Once you have submitted your request, a confirmation page will appear. This page will include a number assigned to your request. Make note of your request number. You may wish to save or print the confirmation page.

Step Two 2. Drop off any associated media.

Many requests have media associated with them. This media might be original cassettes that you would like us to duplicate, a video you would like us to show, or a tape or DVD you would like to put on reserve.

  You may drop off associated media any time the Circulation Desk or administrative offices are open. You must mark the media with the request number you were assigned in Step One. If you wish, you may attach a printout of your confirmation page to the media.

Step Three 3. Receive a confirmation by e-mail.

A member of our staff will send you an e-mail message to notify you that we have received your request and any associated media. If we have any questions about your request, we will contact you by e-mail at this time.

Step Four 4. Receive a completion notification by e-mail.

We will e-mail you when your service request has been completed.


Note: after this e-mail notification has been sent, you have thirty days to pick up your materials. After thirty days, the CLS may, at  its sole discretion, discard the materials, add them to its own collection, or return them to you by campus mail without responsibility for their disposition or value. Items left with the CLS beyond this thirty day notice period that are not discarded will not be available for pick-up without prior appointment.
